Aim of the Course

This course aims to develop the knowledge and practical skills required to audit adult social care records consistently, identify weaknesses and support measurable improvement.

It helps learners to:

1. Understand the purpose and limitations of auditing
2. Establish clear audit standards and criteria
3. Select appropriate records for review
4. Assess completeness, accuracy and timeliness
5. Identify contradictions, omissions and unexplained gaps
6. Review whether records remain person-centred
7. Recognise safety, safeguarding and confidentiality concerns
8. Record audit findings objectively
9. Develop proportionate corrective actions
10. Monitor completion and conduct re-audits
11. Use audit findings to improve care rather than merely produce scores

Scope of the Course

This course is suitable for senior care assistants, team leaders, care coordinators, nurses, deputy managers, registered managers, quality leads and other authorised staff responsible for checking care documentation in England.

It covers paper and digital care records, including:

1. Assessments and care plans
2. Risk assessments
3. Daily notes
4. Monitoring charts
5. Repositioning and nutrition records
6. Behaviour and incident documentation
7. Medicines-related records
8. Reviews and evaluation records
9. Consent and decision-making documentation
10. Communication and escalation records

The course does not independently authorise learners to:

1. Access records outside their role
2. Alter or conceal original entries
3. Retrospectively complete records as if written earlier
4. Investigate suspected abuse, fraud or professional misconduct
5. Make clinical judgements beyond their competence
6. Disclose confidential information unnecessarily
7. Change approved care arrangements without appropriate authority

Learners must follow their organisation’s audit procedure, access controls, data-protection arrangements and escalation processes.

Important practice point: A completed audit form does not prove that care records are safe or that practice has improved. Findings must lead to proportionate action, responsible individuals, realistic deadlines and re-auditing. Serious risks must be escalated immediately.
