Aim of the Course

The course aims to help learners understand the principles of person-centred care planning and their role in delivering care in accordance with an individual's agreed plan, preferences, risks and outcomes.

Scope of the Course

Designed primarily for care assistants, healthcare assistants, support workers, senior carers and similar frontline roles, the course focuses on practical responsibilities rather than advanced managerial or clinical care-plan assessment.

Learners explore how to support choice and independence, follow agreed care instructions, recognise changes, document relevant information and contribute appropriately to care-plan reviews.
